All claims must:
(a) have the appropriate form completed in full.
(b) be supported by original receipts on headed paper.
(c) set out exactly the work/treatment involved.
(d) show the date that each item of expense was incurred.
(e) show the name of the patient.
NOTE: Please do not alter or make any changes to your receipts.
Claims supported by original receipts on headed paper should be
submitted after three months of being incurred.
A Hospital/Consultants fees claim form must be used in respect of inpatient fees.
If injured on duty, the state will pay all expenses, claims should
not be made to the Society. If injured in an accident and a claim can be
made from a third party, no claim should be made from the Society.
Payment of any benefit of the Society is subject to strict compliance with
the Rules of the Society.
